    For theraputic CGM, the following codes are applicable:

  • A4239: Supply allowance for non-adjunctive, non-implanted continuous glucose monitor (CGM), includes all supplies and accessories, 1 month supply = 1 unit of service
  • E2103: Non-adjunctive, non-implanted continuous monitor or receiver 
  • Class II DME (including FreeStyle Libre 2 system and FreeStyle Libre 3 system) does not utilize a modifier
  • KF modifier applies to Class III DME (including FreeStyle Libre 14 day system)
